The following form is our Financial Aid form for the coming school year. Financial Aid forms need to be completed for each student for each school year, no exceptions! Financial situations change, scholarship availability and requirements may change from year to year, so we need new forms filled out for each student that wishes to recieve any sort of financial aid.

The above form needs to be sent, along with the required tax documentation and forms and be turned in by March 30, 2010! The above form and others like in on our site requires Adobe Reader to view and use.

2010-2011 Scholarship Form

The above form is due December 31, 2009 for all students wanting to apply for scholarships for the 2010-2011 school year.

Beginning with students entering St. Francis de Sales High School in the Class of 2006, a $1,000 per year scholarship is offered to any student whose parent is a full-time employee of a Catholic School. Students must maintain a 3.0 cumulative grade point average for renewal of the scholarship from year to year.  Catholic School Employee verification form .
