A number of forms are necessary for application to attend St. Francis de Sales High School. All students who wish to attend need to have the proper forms filled out and returned by the dates listed on each one.

If you currently attend a Catholic grade school in the Toledo area please give this form to your 8th grade teacher or counselor and have them fill it out and send it to St. Francis directly. Please ask your teacher or counselor to have this postmarked by December 14th, 2012.

If you currently attend a public grade school in the Toledo area please give this form to your parent or guardian and have them fill it out and return it to the St. Francis office by December 14th, 2012.
